2010-2011 Enrollment Agreement

SCHEDULE  This agreement is for the school year, August 14, 2010 through May 15, 2011.  Allowing for Thanksgiving, Christmas, Spring holidays and snow days it guarantees 34 lessons including the final dress rehearsal.  Snow days and classes missed because of illness may be made up.   There are no refunds or deductions for missed classes; however, we encourage students to make-up missed classes.  Classes with an enrollment of fewer than six pupils are subject to cancellation.

 

TUITION  ·  Tuition is an annual fee paid in 9 ½ monthly installments.  It is based on the number of lessons in the entire year – not the number of lessons in an individual month.  Tuition is due by the first day of each month and must be kept current.  A $10 late fee will be applied to any tuition or other fees received ten days after the fee is due.  We do not mail monthly statements.  If you receive a bill in the mail, the account is past due.  No student will be allowed to take class if the account is two months past due.  Costumes will be distributed beginning Saturday, May 1, 2010.  Students’ accounts must be current through the end of May in order for students to receive their costume(s).

 

A $25 fee must be paid for any returned check.  If the school receives two insufficient funds checks all future tuition and fees must be paid with cash, cashiers check or a money order.  Except in the case of cancelled or rescheduled classes, registration fees are non-refundable.

 

WITHDRAWALS  · Student may withdraw from any or all classes at any time by giving written notice to the office no later than the end of the month.  If written notice is not received by the end of the month, the next month’s tuition must be paid.

 

RELEASE OF LIABILITY  ·  I do hereby release the Gainesville School of Ballet and its staff from any liability occurring on or around studio premises, or at any function held at other locations in connection with the dance classes in which the below named student is enrolled.  I declare that the student is in good health and can participate in dance education classes.  Given the nature of dance classes, and with the knowledge that injuries sometimes might occur, I have taken the necessary steps to obtain accident, health or hospitalization insurance which would cover any sustained injury.  In the event of an injury or emergency when I cannot be contacted, I give my permission for you to obtain medical services for the below named student.

 

OTHER   ·  I give permission for my child’s picture to appear on the Gainesville School of Ballet or Gainesville Ballet Company website or other publicity.

 

 

Please enroll my child for the 2010-2011 season.  I agree to pay the agreed upon rate of tuition by the 1st of each month.
